She was feared to be losing her battle with motor neurone disease.
She was last seen on Sunday 13 December, when she visited family in Leyton, East London, with Zachary Bilal Kent-Blake, eight, and four-year-old Amon Ben George Kent-Blake.
Her partner and the children's dad Arthur Simpson-Kent, 48, has also been missing since 16 December Scotland Yard tonight announced their Homicide and Major Crime Command are now leading the probe into the search for the four.
DCI Gwyn said: "A number of enquiries are underway to find Sian Blake, Arthur Simpson-Kent and their two children.
"They have all been missing for a considerable period of time now, and I am growing increasingly concerned for their welfare.

"I am appealing directly to Sian and Arthur to get in touch with the police and let us know they are all well. I am also appealing to anyone who has seen the family or knows where they may be to get in touch as soon as possible."
Police broke into the family home on 15 December but found it empty. Sian's car was found abandoned in Calvert Road, Bethnal Green, East London, yesterday fuelling police concerns for their welfare.
Cops said it was not known why or whom parked the silver/beige Scenic – registration number SM54 FUO.
Sian is described as a black woman, 5ft 2ins tall of slim build with short black hair, who often wears glasses.
Arthur is described as a light skinned black man who is 6ft 2ins tall. Zachary and Amon are both described as light skinned black boys.
An appeal last week by the Met to trace Sian and the boys failed to fine any trace of them. Texts from Sian's phone have been received by her relatives saying she wanted to be alone with her kids for her last Christmas.
It is thought that the family may have been in the Colchester and Cambridge areas since vanishing.
SIAN enrolled in acting classes when she was 11 and her telly debut came in an episode of Casualty at Christmas 1994.
But 18 months later she landed her best-known role as soul-singing home-wrecker Frankie Pierre in EastEnders. She appeared in 56 episodes in 1996 and 1997, under her stage name Syan Blake.
She also appeared in The Bill, Doctors and Skins, as well as West End shows including Twelfth Night and The Burial.
